New Empire BlueCross BlueShield ID Cards - (11/25/2019)

Empire BlueCross BlueShield will be sending you a new medical ID card in or around mid-December.  Beginning January 1, 2020, your old ID card will no longer be valid.  Please be extra careful not to overlook this new ID card in the mail as it will be required to receive medical services on or after January 1, 2020.  You should provide this new ID card to your providers so that claims are correctly processed.  After you receive your new ID card, please destroy your old card as it will not be valid beginning January 1, 2020.

Online Services

You may access your online services by registering at any time after your coverage begins. This will allow you to check claim status, locate networks, providers, receive personalized health information and much more.

Customer service

•           Send Empire a secure e-message through Member Online Services at

           Call Empire at the phone number listed on the back of your new member ID card.

Please remember to:

           Always carry your ID card.  Show it to your provider whenever you receive services.  You should proactively inform your providers of your new insurance ID information.

           Report changes in family status promptly.

           Destroy your old card.
